We are contracted with many insurance companies such as Aetna, Blue Cross, UnitedhealthCare, Preferred Community Choice PPO, Cigna and others. Please check with our office staff to see if we are an "in network " provider for your plan. Do not depend on the booklet sent to you by the insurance company as this may be out of date. Co-payments and deductibles will be collected at the time of service. We will file claims for you.
When In Focus Health is not in your network and your policy says you have "out-of-network" benefits, the portion of your visit covered may be somewhat less than if we are "in-network".Typically this may vary from 60 to 70% of the charge plus any co-payment or amount that is part of your deductible. We will need to establish your specific benefits before the visit so that we both know what portion will need to be paid at the time of your visit. We also will file claims for you for the balance of payment.
For those who do not have insurance or for those for whom their insurance plan does not recognize our office as a provider, all fees are payable at the time of service. We do have low monthly payment financing options available. Explanation of these options can be found by clicking the "Payment Option" tab at the upper left of this page.
